What to expect on your first visit to Fletcher Clinic?
When you arrive at our surgery you will be welcomed by our friendly reception staff who will guide you through the registration process. Completing a form with your contact information about yourself and information in regards to your health and family history. 

 

It is requested that if you are a new patient you arrive at least 10 minutes before your appointment time to complete the relevant new patient forms. If you wish you may complete these forms before your appointment you may download these forms below. 
 

Why do you need to fill a new patient form?
If you are a new patient to our surgery these forms give us any relevant information about your health including any family history, pre-existing conditions or other issues. This will give the doctor a better understanding of what could be contributing to your symptoms or concerns.
